Get in

Take a bus to either Phedi or Nayapul. If takes 2h to Nayapul by bus and costs Rs110-200. Please, note there are several bus stations in Pokhara (taxi ride from Lakeside to bus station Rs200).
Taxi on the route Pokhara-Nayapul costs Rs1500 1.5hrs (Nov 2010), Pokhara-Phedi Rs700-1000 20mins (Aug 2013), Pokhara-Dhampus Rs2000 (Aug 2013).
Itinerary

There are a myriad of trails that traverse the Annapurna and lead to the Annapurna Base Camp.
Nayapul to Ghorepani and Poon Hill
Nayapul is marked by a collection of road side shacks that function as bus/taxi stop backed by a slightly more substantial village that has a jeep track through the middle of it.
Start your trek by following the jeep track down until you spot a swingbridge to your left, there is a foot path leading to it and you cross the river here. Turn left as you come of the bridge and pick up the track along the Modi Khola for about half an hour before you come to the outskirts of Birethanti and a ACAP check point where you have to get your Conservation permit checked. You can now see another swing bridge that you have to cross. Directly opposite the swing bridge is the TIMS office where you can either buy TIMS certificate or just check it if you have it already.
Birethanti has a number of lodges although if you have just started your hike you may wish to walk on to Tikhedunga.
Keep the river to your left hand side as you follow the start of a jeep track, and the destruction of the trail, for an hour or so before it peters out at the small hamlet of Baajgara where there are two tea shops. The trail keeps following the river, make sure the river is always to your left, and eventually you arrive in Hile where there are four lodges. The lodges owners here like to make out you are already in Tikhedunga in order to catch some of their customers. In fact Tikhedunga and its myriad of lodges is another fifteen minutes down the track.

Silvana Pagani from Italy wrote in 2011 about my experience in Nepal and recommends as guide, Sanjib Adhikari.
This year at the end of february I went in Nepal for 18 days for the second time and I did a small trekking in the Annapurna region, I went to visit Pokhara, Kathmandu, Bakthapur and Nagarkot and I saw the National Park of Chitwan and in this trip I had the pleasure to meet Sanjib Adhikari.
He is an independent trekking guide and tour operator in Nepal and he has already obtained trekking guide license from the ministry of tourism, government of Nepal.
In his work Sanjib is a person very responsible and serious, very helpful and attentive to his customers, he knows very well the mountain because he has many years of experience and is also an excellent guide for visiting the cities.
He is someone very competent in his work and always ready to find the best solution, if necessary and speaks English very well.During the trekking and the tours he will tell you many interesting things about culture, about life in the villages, mountains, history and traditions in Nepal.
The best thing is that Sanjib loves his country very much and loves his work and always manages to convey all his enthusiasm with simple ways and always with the smile. He is a very nice person and is always attentive to the needs of his customers. Sanjib is also very funny and after this experience we became a very good friend. Next year at the end of January I will return for the third time in Nepal and Sanjib will be again my guide. .
For these reasons I am very glad to report all references of Sanjib.
